The Australian Prudential Regulation Authority has developed and released electronic forms to support entities in meeting the notification requirements under Prudential Standard CPS 230 Operational Risk Management. The forms cover notifications of an operational risk incident (paragraph 33), a breach of critical operation tolerance (paragraph 42), and a new or changed material arrangement and or offshoring (paragraph 59(a) and (b)).
