The Australian Prudential Regulation Authority (APRA) has released electronic forms for regulated entities to use when meeting the notification requirements under Prudential Standard CPS 230 Operational Risk Management. The forms cover notifications of operational risk incidents (paragraph 33), breaches of critical operation tolerance (paragraph 42), and new or changes to material arrangements and/or offshoring (paragraph 59(a) and (b)).
