The change adds more personal identifiers to the Authorization Form for Querying so the central bank can better match individuals against its records and determine whether they are qualified or disqualified to hold positions in Bangko Sentral supervised financial institutions. The amended form now requires the complete maiden name of a married female, taxpayer identification number, birth date, sex and last known address of the person subject to the query. The circular says the added information provides an extra verification layer against existing Bangko Sentral records and aligns the form with existing rules requiring supervised financial institutions to use the Authorization Form for Querying. The annexed form also indicates that a valid passport number may be used if no taxpayer identification number is available. The circular takes effect 15 calendar days after publication in the Official Gazette or in a newspaper of general circulation.
